The operation is only half the job. What happens in the months afterwards largely decides how strong, confident and ready for sport you’ll be. Our ACL and post-surgery rehabilitation is led by Laura Bhreathnach and built on clear milestones, objective strength testing and the gym equipment in our Ballsbridge clinic.

Your ACL journey, stage by stage

Every surgeon has their own protocol, and we work within it. Progress is based on meeting clear criteria, not simply on how many weeks it has been since your operation.

  1. Pre-hab (before surgery). Reduce swelling, restore full knee straightening and build strength. People who go into surgery stronger tend to recover faster.
  2. Early recovery. Control pain and swelling, get the quad working again and regain range of movement, especially full extension.
  3. Strength. Progressive gym-based strengthening of the quads, hamstrings, glutes and calves, with regular testing to track the gap between your legs.
  4. Running & jumping. A graded return to running, landing, hopping and changing direction once your strength targets are met.
  5. Return to sport. Sport-specific training and a final test battery. Returning to pivoting sports usually takes at least nine months, and the decision is made on test results, not the calendar.

Why measure everything?

How your knee feels is not a reliable guide to whether it’s ready. Many people feel “fine” long before their strength has returned, which is one reason re-injury is a real risk after ACL surgery. We regularly test strength and hop performance and compare your operated leg with your other leg, so you can see your progress in black and white and know exactly what’s left to do.

Post-operative rehabilitation and knee injuries are two of Laura’s special interests. Her background in elite sport, with Scottish Rugby and the Scottish Institute of Sport, shapes a rehab process that is structured, measurable and designed to get you back to the activities you love, not just back walking.

Post-surgery physiotherapy with a Chartered Physiotherapist can be claimed on most Irish health insurance plans. Some plans offer extra benefits after surgery.

When should I start physio after ACL surgery?

Usually within the first week or two after your operation, following your surgeon’s guidance. Ideally, start pre-hab before surgery too.

How long does ACL rehab take?

Most people need at least nine months before returning to pivoting sports such as rugby, GAA or soccer. The exact timeline depends on meeting strength and function criteria.

Can I rehab an ACL injury without surgery?

Some people can, depending on their knee, their sport and their goals. We can assess you and discuss the options alongside your consultant’s advice.

Will you follow my surgeon’s protocol?

Yes. We work within your surgeon’s protocol and restrictions and are happy to liaise with your medical team.

Do you rehab knee and hip replacements?

Yes. We help you regain movement, strength and confidence after joint replacement, so you can walk, climb stairs and get back to activities like golf and walking holidays.
